Opens up a dialog box which reads I want to read a(n) <1st choice> story that's <2nd choice> with/that has a <3rd choice> twist!

Each of the choices can be switched through up to 3 different options by using the firing buttons, and will have a different effect on the final book that can be created using the Write! button, which grants the My Story item.

The 1st choice determines the book length, i.e. the amount of charges the item has (short = 2, average = 4, long = 6). The length of the book can determine the strength of some effects.

The 2nd choice and 3rd choice determine the effects of the book. The same option can appear for each of the 2 choices, but one book can never have both of them at once.

Once the dialog box has been opened, it cannot be closed unless the book is written. The game will not be paused while writing the book.

The possible effects are as follows. If multiple options are listed, it means that they depend on the book's length (short, average or long) respectively.

Festering
Spawns 1, 4 or 6 Blue Skuzzes.

Frightening
Shakes the screen and inflicts fear to enemies in a short radius (or all enemies if not a short book) for 5-8 seconds.

Funny/Full of jokes
Isaac produces a normal fart, a poisonous fart, or 8 poison farts around him.

Love
Spawns a Half Red Heart, a Full Red Heart, or a Soul Heart.

Mischief
Spawns 1-3 Hollow Friend minions.

Profitable
Spawns 2 pennies (short book), or 1 penny and 1 random coin (average book), or 1 penny, 1 random coin, and 1 random pickup (long book).

Religious
Spawns 1-3 Book of Virtues wisps.

Sad
+0.25, +0.5 or +0.75 tears for the current room.

Shocking
Shakes the screen and inflicts 1-3 stacks of Bruising to enemies in a short radius (or all enemies if not a short book) for 5-8 seconds, that causes them to take +1-3 extra damage from all sources.

Violent
Shakes the screen and damages enemies in a short radius (or all enemies if not a short book) for damage equal to 15 + CurrentStage × 3, 20 + CurrentStage × 3, or 30 + CurrentStage × 4.

Wild
Randomly does one of the previously listed effects.
This item contributes towards the Bookworm transformation. Collecting 3 items from this set will make Isaac fire an extra tear 25% of the time.
